This installation was a pretty straight forward, except for one minor point that was left out of the documentation. During step 9 you want to make sure your wordpress post is set to HTML and not Visual.
9. With the page file open, add this code <!--gb_contact_form--> to the text area and "Save" the page. For alternatives, please check out this FAQ and this other FAQ above.
